Share pertinent patient information to ensure accurate evaluation by the multidisciplinary team.
Qualifications:
Associates, Bachelor's, Master's, or Doctoral degree from an accredited physical therapy program.
Active state license in Physical Therapy / Physical Therapy Assistant.
